2026 Blue & Green Day Contest Submission Form

  • Thank you for supporting organ, tissue & eye donation through participating in Donate Life California’s Blue & Green Day celebration and contest. April is National Donate Life Month, and it’s time to show your Blue & Green Day spirit!!! This year’s theme is Leave a Legacy

    Please fill out the form below and upload your photo to each category you’d like to enter. You can only send one photo per category.

    DMVs in the following categories will be recognized as winners:

    • Best Blue & Green Day Legacy Story Photo Statewide (Photos are judged throughout the entire state of CA)
    • Facebook Favorite Statewide (All photos are posted on Donate Life California Facebook page and the photo with the most likes will win)

    If you have more photos you’d like to share (we’d love to see them) they must be sent separately from your contest submission and will not be part of the contest. Feel free to email them to stephanie@donatelifecalifornia.org

    Photo submissions due by 11:59pm PST on Friday, April 24th.

Your story should share one or more of the following: How donation has made an impact in your office, a moment or message that inspired your team, a personal connection from a staff member, or a story shared by a customer that touched your workplace.
  • Inspire others with your story! Share a photo that captures the story above. Your photo should visually communicate the story, emotion, and heart behind your office’s connection to the cause.
